69yr old male with c/o of dov in right eye

69yr old male came to opthomology with chief complaint of dov in right eye since 5 months.
HISTORY OF PRESENT ILLNESS 
Patient was apparently asymptomatic 5 months back, then he developed dov in right eye which was insidious in onset and gradually progressive.
HISTORY OF PAST ILLNESS

Dm(-) 
Asthma(-) 
PERSONAL HISTORY 
Married 
Occupation: farmer
Appetite:normal
Diet:mixed
Bowels are regular 
Micturition is normal 
No known allergies 
Habits: alcohol occasionally, tobacco 
FAMILY HISTORY 
Not significant 
PHYSICAL EXAMINATION 
No pallor, icterus,cyanosis, clubbing of fingers or toes , lymphadenopathy,malnutrition 
pulse rate: 81/min
Respiration rate: 19/min
Bp: 120/70 mmhg
Spo2:97% 
SYSTEMIC EXAMINATION 
Abdominal examination 
Shape: scaphoid
No tenderness 
No palpable mass
Normal hernial orifices 
No free fluid
No bruits 
Liver and spleen are not palpable 
Bowel sounds:yes 
RESPIRATORY SYSTEM 
No dyspnoea, wheeze 
Position of trachea is central
Breath sounds vesicular
CNS-NAD
CVS
No thrills , cardiac murmurs 
Cardiac sounds:S1 S2


Provisional diagnosis
Senile mature cataract (smc) in right eye
Investigations:rbs 
Medication: Eld.ciplox BD
