60yr old female with lipoma over medial aspect of right knee

A 60yr old female came to hospital with swelling over medial aspect of right knee since 3years 

HISTORY OF PRESENT ILLNESS 

Patient was apparently asymptomatic 3years back and then she developed a swelling which is insidious in onset and gradually progressive. Patient has no history of pain,no discharge,weight loss,fever ,trauma 

HISTORY OF PAST ILLNESS 

Patient has no history of hypertension, diabetes mellitus,asthma,epilepsy,cardiovascular disease

PERSONAL HISTORY 

Married 

Apetite: normal 

Diet:mixed 

Bowels:regular 

Micturition:normal 

No known allergies 

No addictions 

No known family history

GENERAL EXAMINATION 

No pallor , no cyanosis,lymphadenopathy,malnutrition,icterus,clubbing of fingers or toes ,edema of feet 

VITALS:

Temperature: 98.7F 

Pulse rate:80/min

Bp: 100/70mmhg 

Spo2:99percent 

SYSTEMIC EXAMINATION 

CVS

No thrills , cardiac murmurs

Cardiac sounds:S1S2  

RESPIRATORY SYSTEM 

No dyspnoea, wheeze 

Position of trachea:central 

Breathing sounds: vesicular 

ABDOMEN 

Shape of abdomen:scaphoid 

No tenderness,palpable mass, free fluid, bruits,bowel sounds 

Normal hernial orifices 

Liver and spleen not palpable 

CNS 

Level of consciousness:conscious 

Speech:normal 

CLINICAL FINDINGS

Sloitary, oval swelling located at medial aspect of right knee joint . The surface of swelling is smooth the swelling was non pulsatile . There was no local rise of temperature, firm in consistency,was freely movable and there was no fixation to underlying skin . Skin over the swelling was pinchable 

PROVISIONAL DIAGNOSIS 

Lipoma over medial aspect of right knee 

Medication:

Tab.pan40mg 

Tab.MVT

Patient was advised to have excision
